Dear New & Returning Families Enrolled for 2022-2023,
Medical and athletic forms for the 2022-2023 school year are now available in Shady Side Academy's Magnus Health system. Please read the instructions below for accessing and completing the required forms by August 1, 2022.
Accessing Your Magnus Health Account
Your SSA Magnus Health account is linked to your Veracross portal account. Log in to Veracross at http://accounts.veracross.com/ssa/portals/login and click the gold Magnus Health button on the Parent or New Family Portal to be automatically redirected to your Magnus Health account.
Note: New parents will land on the New Family Portal upon login until the system rolls over for 2022-2023 in late June. After rollover, new parents will land on the Parent Portal upon login. Faculty/staff parents may land on the Faculty Portal and can toggle to the Parent or New Family Portal by clicking the Veracross icon at the top center of the screen.
Completing Forms in Magnus Health
Once in Magnus Health, you should see your SSA student(s) listed on the Front Desk page with a note indicating you have requirements on your To Do list. Click the orange "Complete Now" button to access and complete the child's required forms. (Note: Parents of graduating seniors, please disregard any "To-Dos" flagged for your senior. The Class of 2022 will be archived in Magnus Health after Commencement.)
Most forms can be completed online directly in Magnus Health. Forms that require a physician's signature must be printed, completed and signed by a physician, then scanned and uploaded to your Magnus Health account. Do not submit any paper forms to SSA. Physical exams requirements are as follows:
- Grades PK-5: The SSA PK-5 Physical Exam Form, signed and dated by a physician after Aug. 1, 2021, is required for all PK-K students and new students in grades 1-5.
- Grades 6-12: The PIAA Section 7 Physical Exam Form, signed and dated by a physician after June 1, 2022, is required for all students in grades 6-12. Parents are encouraged to schedule summer physicals now.
In mid-July, you will begin receiving an automated weekly reminder email from Magnus Health for each child with outstanding forms.
All forms are due August 1, 2022. Students missing required physical exam forms or immunization records will not be permitted to begin classes or preseason athletics until forms are received.
