I hereby approve of my child's attendance and participation in the activities of Shady Side Academy Discovery programs and certify that he/she is in good health and able to participate in all program activities. I authorize that the directors act for me according to their best judgment in any emergency requiring medical attention for which service I will pay. I hereby waive and release Shady Side Academy and its officers and subcontractors from all liability for any injuries or illness incurred while at camp.
A photographer will be on site to photograph program participants. I consent to the use, publication, and copyright by Shady Side Academy, in school publications, brochures, and other promotional materials of photographs in which the student may appear as a student in any programs or activities of Shady Side Academy. |
